Wholesale Distributor Application

Please download the below files and complete the Distributor/Wholesale Agreement and return it to us by one of the methods mentioned in Wholesale Distributor Cover Letter.

We ask that the first order be paid with a credit card (Visa, MasterCard, American Express, or Discover). You can enter your credit card number on the agreement form. If after the first order, you wish to set up net credit terms with us please be sure to complete the reference section of the agreement. If you have an existing account with Innersource, you need not complete question #10 on the Wholesale Agreement.

Your wholesale discount can be used for the items listed on the Price List. You may notice there are some items for sale on the website (package discounts, advanced DVD sets, etc.) that are available at retail prices only, because they are already discounted or are not something most would resell. A list of available products with wholesale pricing is attached. When ordering, please contact me directly.
